مؤسسة ترانس-فوزيا هي مؤسسة بولندية تأسست عام 2008 وتدافع عن حقوق الأشخاص المتحولين جنسياً وغير الثنائيين ومزدوجي الجنس وغير المطابقين للنوع الاجتماعي. تعمل على تحسين جودة الحياة من خلال مكافحة التمييز، وتقديم الدعم النفسي والقانوني، وإجراء حملات تثقيفية ومكافحة رهاب التحول الجنسي. تدير المنظمة بشكل أساسي أشخاص متحولون جنسياً وغير ثنائيين، وتخدم المجتمع بالإضافة إلى المتخصصين والمؤسسات.
